The Woods Medical Centre offers individualised care plans (a.k.a. GP Management Plans) for people with chronic disease, including Diabetes.
Chronic Disease
Chronic disease, as defined by the Australian Institute of Health and Welfare (AIHW), are ‘long lasting conditions with persistent effects.’
These types of diseases are becoming increasingly common in Australia, with 1 in 2 Australians suffering from at least 1 of 8 chronic conditions in 2017-18 (See The Australian Institute of Health & Welfare website for more information).

Arthritis (Osteoarthritis)
Arthritis (Osteoarthritis) is the swelling and tenderness of joints. Joint pain and stiffness typically worsen with age. The most common types of arthritis are osteoarthritis and rheumatoid arthritis.
